mOTivated kids LLC is an out-of-network provider.  We do not accept insurance at this time. 


You may request a special billing receipt (superbill) to submit to your insurance for reimbursement, depending on your plan and benefits. Superbills are generated at the end of each month and can be submitted to your insurance provider, where applicable.


​We encourage all families to contact their insurance provider to determine whether out-of-network services are reimbursed, and if so, to identify whether your provider requires you to provide specific documents (i.e. a prescription from a pediatrician, diagnosis code, letter of necessity, etc). This will help us provide you with necessary information to speed up the time in which you are reimbursed.


​All payments are applied to your card (debit, credit, or FSA or HSA card) on file through our secure HIPPA compliant online portal, Simple Practice.​​
